Please include the patient’s name, telephone, social security number, date of birth, physician’s name, name of medication to be refilled, last refill date, pharmacy name, telephone and fax numbers. Faxes and messages are retrieved throughout the business day. If you need to speak to the clinic immediately, press ‘1’, otherwise leave a message after the tone.
