Application Information for Academic Credit

To apply to the program for online blood bank technology courses for academic credit, please follow these steps in the order in which they appear. (It may be helpful to print this page.)


1. Apply for admission to the program using The UT Southwestern School of Health Professions Online Application. Please submit your application before the deadline. This application process is required prior to taking the first course in the program.  Complete the application and supplemental application online.  Send the application fee and required documents to the address on the application form.

Three letters of Recommendation 
Official Transcripts
Foreign Transcript Evaluation (foreign applicants)
TOEFL Score (foreign applicants)

2. Once you have been admitted to the program, you may want to apply for a loan for your tuition.  For students enrolled for academic credit, government loans may be available through The UT Southwestern Financial Aid Office.  Please apply as soon as possible. It can take up to six weeks to approve and process your loan and receive your check.

3. Some employers also provide tuition reimbursement.  It is the student’s responsibility to determine the criteria required by his/her employer for reimbursement.

†Some fees are currently waived for distance students who will not use on-campus services.  Applicants for the Blood Bank Technology Program in the Dallas Metroplex who, if accepted, plan to use on-campus services, attend on-campus laboratory classes, and who want the Education Coordinator to coordinate their rotations must pay these fees.
Additionally, students should expect to spend approximately $1000 on required textbooks over the 2-year period.
