Q: How do I apply for  a transplant at the University Transplant Program at Parkland?

A: Ask your dialysis nurse for an application for the University Transplant Program at Parkland.  Complete the form and ask your nurse to mail it to us.  We will review the application and contact you with the next step.  If you have questions, please call at (214)- 590-5656.
