The evaluation method for each learning objective is listed in the tables below using the following key:

CE = Clinical Evaluation Form
FPP = Formal Patient Presentation Form
D = Didactics
NBME = National Board of Medical Examiners Family Medicine Exam

Goal A: Develop knowledge and skills in the evaluation and management of common primary care complaints.
Learning Objectives and Evaluation Method:
A1.
Conduct a focused history and physical examination . CE
A2.
Present the patient's case, verbally and in writing, in a succinct and organized manner. CE
A3.
Propose an appropriate differential diagnosis, arrive at a reasonable preliminary diagnosis, and initially manage common primary care illnesses. CE, NBME
A4.
Demonstrate sound clinical judgment in working through a patient work-up and development of a management plan. CE, FPP
A5.
Demonstrate an adequate fund of medical knowledge as related to common primary care illnesses. CE, NBME
A6.
Address patient issues regarding health promotion and disease prevention. CE, FPP
A7.

Propose a strategy for treatment and follow-up that:

  1. is supported by the literature
  2. is acceptable to the patient
  3. takes into consideration the patient's social, cultural and economic circumstances
  4. is cost effective and efficient

Goal B: Understand a team-based, patient oriented model for ambulatory care.
Learning Objectives and Evaluation Method:
B1. Communicate effectively with patients and families while gathering information and proposing recommendations for care. CE
B2. Demonstrate sensitivity to patients of differing backgrounds. CE
B3. Collaborate well with all members of the health care system, including clinic staff, nursing staff, colleagues, and consultants. CE
Goal C: Appreciate the psychosocial dimension of care and the importance of professionalism.
Learning Objectives and Evaluation Method:
C1. Gather a full social history, including patient marital status, socioeconomic status, habits, employment, family network, and an appreciation of psychological and spiritual concerns. FPP
C2. Appreciate the cultural, socioeconomic, and other factors affecting the treatment of patients. FPP
C3. Demonstrate empathy and the appreciation of the impact of illness on patients and their families. FPP
C4. Demonstrate taking an active responsibility for one's own learning by asking questions, seeking feedback, and fulfilling all clerkship obligations. CE
C5. Display a courteous and cooperative attitude. CE
C6. Manage time to complete assigned duties, demonstrating a respect for the needs of co-workers, clinic staff, and supervising physicians. CE
Goal D: Demonstrate the ability to effectively communicate clinical information.

D1. Research a common preventive medicine issue in primary care and provide a succinct summary of the topic s applied to a clinical setting. D, FPP

D3.

Adequately prepare a formal patient presentation, including the following elements:

  1. medical history and physical exam
  2. social history
  3. differential diagnosis
  4. diagnostics related to a chief complaint
  5. knowledge of a particular disease process
  6. assessment and patient management plan
  7. preventive medicine and patient education issues
