CLINIC MISSION STATEMENT

It is the mission of the Prosthetics-Orthotics Clinic to provide quality care for our patients and the medical community in an efficient, caring, and professional manner.  In addition, it is our mission to keep the medical community informed of new technology through continuing education and research.  Quality and integrity are at the heart of the care our ABC credentialed faculty provides, with the goal of improving the functional outcome of our patients.

MAKING AN APPOINTMENT

To schedule an appointment, please call 214/645-8250.  We request that you let us know at least 24 hours in advance if you are unable to keep your appointment.  Regular office hours are from 8:00 a.m. to 5:00 p.m., Monday through Friday. Phones will be answered after hours by our answering service.  If you have an urgent need the on-call practitioner will be contacted.

PHYSICIAN REFERRALS

Referrals from your physician can be made by letter, telephone, or FAX.  We request that all pertinent medical records be made available before your initial visit.

TELEPHONE CALLS

Although our office receives a high volume of telephone calls each day, we make every attempt to return your call as promptly as possible.

INFORMATION NEEDED TO BEGIN CARE

Please provide the following information: Patient’s name, phone number, social security number; insurance carrier and policy number; name, address, and phone number of referring physician.   Please bring pertinent medical records from the referring or previous physician.

PARKING INFORMATION

Parking is available for $2.00 in the parking garage adjacent to our building.  Tokens are available for those who have a handicapped parking license plate or hangtag.  Please see clinic staff for details.

UNIVERSITY BASED PRACTICE

Our services cover the entire breadth of prosthetic and orthotic care and are delivered by highly skilled practitioners—all of whom are on the faculty of UT Southwestern Medical Center.  Occasionally a student may be present to observe the faculty.  Students will not render services; their participation is strictly observational.

VOLUNTEER MODEL FOR ACADEMIC PROGRAM

The academic program welcomes your participation as a patient model.  If you have an amputation and would like to volunteer as a model, please let your practitioner know.  These real life experiences are an essential part of the teaching program.
