If needed, discuss project with the IRB coordinators (214-648-3060). Determine the need for a consent form.
Send one copy of IRB NR-1 EXP Form, project summary, consent forms (if applicable), and HIPAA form(s) to the IRB mail code 8843.
Within 15-20 business days the IRB Coordinators will send a letter indicating approval or send an email to the research coordinator or principal investigator indicating requested changes.
Provide the IRB approval to the appropriate authorities of the institutions where the research will be conducted along with their requested research forms (see investigator's manual for respective performance sites).
IRB approval of this research lasts 12 months.
