There are 3 different groups of Exchange Visitors who come to UTSW. Each group requires different processing. Please choose the category most appropriate to you from the selections below.
NEW J-1 SPONSORED BY UT SOUTHWESTERN FOR RESEARCH
Items Needed from Visitor
1. ) J-1 Application Form
- This is an on-line application that the Exchange Visitor must complete and submit.
2.) Curriculum Vitae
3.) English copy of the health/repatriation insurance from home country declarations page with the coverage amounts converted to US dollars
4.) Original funding document, in English, showing dollar amount and dates of funding.
- Only if Exchange Visitor is being paid by an organization other than UT Southwestern
5.) Copy of degree or letter from Dean of university stating that all degree requirements have been met.
Items Needed from Department
1.) J-1 Visa Departmental Application(.pdf format)
- This application is a document that the Exchange Visitor' s UTSW department will submit in order to be considered for the J-1 visa.
2.) Clinical Observation Statement(.doc format)
- Supplemental document for visitors who are coming for Clinical Observation only.
3.) 5 Point Letter(.doc format)
- Supplemental document for visitors who will have hands-on patient contact.
CURRENT J-1 TRANSFERRING TO UT SOUTHWESTERN FROM ANOTHER INSTITUTION
Items Needed from Visitor
1.) J-1 Application Form
- This is an on-line application that the Exchange Visitor must complete and submit.
2.) J-1 Transfer of Program Sponsorship Form(pdf format)
- Exchange visitor should give this form to their current International Office.
- Additionally, Applicant should submit by fax (214-648-2102) to the UTSW International Office:
- copy of all previous DS-2019 forms.
- copy of identification page of passport
3.) Curriculum Vitae
4.) English copy of the health/repatriation insurance from home country declarations page with the coverage amounts converted to US dollars
5.) Original funding document, in English, showing dollar amount and dates of funding.
- Only if Exchange Visitor is being paid by an organization other than UT Southwestern

ECFMG-SPONSORED J-1 CLINICAL TRAINEE
Items Needed from Visitor
- Visitor goes to the ECFMG website to download and complete applicable forms then submits them to the UT Southwestern International Office for processing.
