With the exception of UT Southwestern checks, please submit a complete packet including all documents from department and applicant simultaneously. UT Southwestern checks should be requested as early as possible and provided separately as soon as ready.  A completed copy of the checklist, below, should be attached to the top of the packet.

H-1B Classified Employee Application form (hiring department must send to HR  Compensation)

H-1B Agreement to Pay Costs Home (prepare on departmental letterhead)

 
____ 4. Letter from supervisor, which outlines job duties. This brief (1-2 paragraphs) letter should incorporate the term "specialty occupation" and outline the minimum degree required for the position.
____ 5. For M.D.'s with patient care responsibilities: copy of Texas license, evidence of passage of USMLE (parts 1,2,& 3), and oral and written English ability (ECFMG certificate)

____10. Copy of final degree with English translation
____11. Equivalency evaluation, if foreign degree. See H-1B Credentials

Copy of biographic and validity page(s) of passport and current I- 94 (if any)

Copies of previous U.S. immigration status documents (e.g. I-94, I-20, DS-2019, I-797)

Waiver of two year home residency requirement (if applicable)

If currently in H-1B status, a current employment letter or recent paycheck stub.
 
If you have dependents in the U.S. who will change status please submit:
Copies of the identity page, validity page and visa page of their passports and their I-94 forms.
USCIS I-539 Form signed by the primary dependent (usually spouse) and supplement will be completed for all other dependents.

NOTE: Do not travel outside the U.S. while your petition is pending at Immigration. Doing so will cause your petition to be cancelled.
