If you change your personal address or telephone number, you are required by Federal Law to complete the following forms to report these modifications within 10 days:

• Change or Update your current information on OASIS*
• Notify your current department of the change(s)
• Submit an On-line AR-11 form to Department of Homeland Security (DHS)
