Children's Medical Staff Services outlines a specialized process for obtaining medical/dental staff membership and clinical privileges. Children's works closely with departments at the university to ensure that the practitioner experiences an expedient and proficient credentialing process.
Children's requires that an applicant requesting privileges must first undergo the pre-screening process by submitting a Provider Request Form "PRF" and curriculum vitae to Medical Staff Services (For more information regarding obtaining a PRF and/or what it entails, please review the FAQ's). Once an applicant undergoes the pre-screening process, a Children's application packet, clinical privileges, and CMC specific instructions will be sent to the applicant and/or department contact. Please note that if an application is sent to Children's prior to pre-screening it will be returned. All required documents must be completed, compiled and submitted to Children's. Department contacts may deliver application packets and additional documents to the practitioners for completion and the practitioner may return completed packets to their department contacts to be forwarded to Children's Medical Staff Services. Department contacts should work closely with the practitioner to ensure that the credentialing checklist is completed prior to the return of the packet to Children's Medical Staff Services. Again, all returned packets must be complete with all necessary documents attached to be processed or will otherwise be deemed incomplete and will be returned.
Upon receipt of a completed initial application packet Children's will send information regarding required online training. Completion of the online training will be due prior to receiving medical/dental staff membership and clinical privileges. Instructions and login information will be included that will grant access to Children's intranet (ChildNet) that will allow you to review and print important documents such as the constitution, rules and regulations, medical/dental staff policies, etc.
