Referring a Patient

We welcome referrals of women with complex or refractory obstetrical or gynecological problems.  Referrals can be made by letter, telephone, or telefax.  In fact, referring a patient is as simple as scheduling an appointment and faxing us the pertinent medical information.  If you want to speak to a physician personally about a patient, please call one of the Contacting a Physician numbers and select option 9.  You can make an appointment for a referral patient by calling the same number and selecting option 1.  For a complete list of fax numbers, CLICK HERE.

 



Information Needed to Begin the Referral Process




Please be prepared to provide the following information:

  • Patient's name

  • Patient's phone number

  • Social Security number

  • Insurance carrier and policy number

  • Name, address, and phone number of referring physician

Also requested are pertinent medical records from the referring or previous physician including:

  • Pathological specimens and reports

  • X-ray films and reports, if available

  • Date of patient's last menstrual period if the patient is pregnant



Special Information for New Patients


New patients should plan to arrive one-half hour before their scheduled appointment to complete the necessary registration forms.
