Combined use of superficial laser peel and nonablative dermal remodeling in a pig model

Laser skin resurfacing of aged and photo damaged facial skin has gained great popularity in the past decade. However, postoperative morbidity and unwanted side effects of the procedure motivated the patients and the doctors to seek less invasive methods at the expense of suboptimal clinical outcomes. We hypothesize that the two unique and minimally invasive skin rejuvenation modalities, which are superficial erbium:yttrium-aluminium-garnet (Er:YAG) laser peel and nonablative laser-induced dermal remodeling, can be used in the same treatment session to take the full advantages of each method without increasing postoperative morbidity and side effects. No study has been reported about the combined use of these two minimally invasive techniques to address the epidermal and dermal abnormalities simultaneously. Our treatment strategy is to remove the superficial epidermis with a purely ablative Er:YAG laser, to leave the basal layer with a 30- to 50-µm-deep papillary dermis intact as a buffer zone, and to treat the deeper dermis using 1320 nm neodymium: :yttrium-aluminium-garnet (Nd:YAG) laser with dynamic cooling system. The purpose of the study is to evaluate the wound healing properties and side effects of superficial Er:YAG laser peel combined with a nonablative dermal remodeling system in a porcine model and compare with that of each modality as well as two different laser resurfacing methods.
