To refer a patient
Physician referrals of patients can be initiated by telephone, e-mail, fax, or mail to the information listed below. Referrals may be made Monday through Friday, 8:00 a.m. until 5:00 p.m.

Information needed for referral:
 • Patient's name, address, phone number
 • Date of birth (if applicable)
 • Pertinent medical information
 • Insurance information (if available)
 • Referring physician's name, address, telephone and fax numbers
 • Reason for referral, area to be evaluated, test(s) being ordered.
After you make a referral
A UT Southwestern employee will call your patient to pre-register him or her and to schedule an appointment.  Or, the patient will be asked to fill out registration forms when they arrive for their first appointment.
Information needed to begin:
 • Patient's name
 • Address
 • Date of birth
 • Social security number
 • Home and work phone numbers
 • History/symptoms/diagnoses; reason for exams; area to be evaluated
 • Prior medical records or address(es) to request records
 • Insurance information
 • Referring physician's name, address, telephone and fax numbers
Information from previous evaluations and X-rays/scans should be sent prior to or should accompany the patient at the time of consultation.
If you would like to fill out your paperwork before your consultation, you may print the "Patient Confidential Health Questionnaire" to complete and bring with you from the link provided.
When the examination and/or other studies are complete, you will be notified of the results.  We encourage your continued participation in your patient's care.
