| Physician referrals of patients can be initiated by telephone, e-mail, fax, or mail to the information listed below. Referrals may be made Monday through Friday, 8:00 a.m. until 5:00 p.m. |

| After you make a referral |
| A UT Southwestern employee will call your patient to pre-register him or her and to schedule an appointment. Or, the patient will be asked to fill out registration forms when they arrive for their first appointment. |

| Information from previous evaluations and X-rays/scans should be sent prior to or should accompany the patient at the time of consultation. |

| When the examination and/or other studies are complete, you will be notified of the results. We encourage your continued participation in your patient's care. |
