1. When do I fill out the Provider Request Form (PRF) for incoming faculty?
Answer:
As soon as the employment offer is extended and the Chair has completed the New Recruit Credentialing Designation Form.
2. Is it really important to mail the introduction letter from the chairman to the practitioner?
Answer:
It is essential!! UTSHS credentialing allows a practitioner 60 days (from the date the introduction letter is sent) to remit the reply form to UTSHS indicating that licensure applications have been completed and submitted. If the recruit has not submitted the licensure application within 60 days, the Vice President for Clinical Operations will contact the department Chairman to inform of the practitioner's lack of response.
3. How do we know which TDI application belongs to UTSHS?
Answer:
We have labeled our TDI application in red at the top of each page. The label reads "UT Southwestern Health Systems".
4. How do we as clinical departments stay updated regarding the status of our providers?
Answer:
The Primary Source Verification (PSV) report is sent to Clinical departments twice a month by the Credentialing Supervisor and tracks the practitioners' progress in the credentialing process.
5. How do I check the status of my practitioners being loaded by the MCOs into their databases?
Answer:
Access the UT Southwestern website /www8.utsouthwestern.edu.
Go into Managed Care Guidelines.

6. What if our new practitioner arrives and we are still waiting for them to submit an application for Texas licensure?
Answer:
If a practitioner has not obtained necessary Texas Licensure and completed UTSW paperwork in a timely fashion, a letter will be sent from the Vice President for Clinical Operations on the individual faculty member. A faculty temp license is not acceptable after 90 days of employment.
7. Now that my practitioners are credentialed and loaded on payer panels, do I have to think about credentialing any more?
Answer:
Yes, practitioners are recredentialed every 3 years.
8. How will I know if my practitioners are due for recredentialing?
Answer:
When a practitioner is approaching a recredentialing due date, a pre-populated Texas Standardized application packet is hand delivered by UTSHS Credentialing staff to the department for review, update and signature. Timely completion at recredentialing is just as crucial as at initial credentialing. When the application is complete, call UTSHS Credentialing to pick up the packet. Do not interoffice mail packets.
9. Are there practitioners who require special processing?
Answer:
Yes, mid level practitioners such as nurse practitioners, physician assistants,
PT, PhD, etc. employed by UTSW.
NOTE: If mid levels are employed by hospitals, billing enrollment or credentialing for managed care payers is not handled by the UTSHS Credentialing staff.
10. How do I terminate a practitioner who leaves UTSWMC, retires, or changes to a research only status?
Answer:
•Complete the PRF and indicate appropriate changes.
•Submit PRF form to the names listed on PRF.
•Remember - only term the parts of the process you want eliminated for a provider. (Billing Enrollment, Managed Care Credentialing, hospital) the practitioner may term from one but not the others.
NOTE: If a practitioner terminates from Managed Care participation for longer than 30 days, the practitioner must be processed as a new practitioner to become reinstated.
