Termination PRF's

 

When a practitioner leaves UT or changes status, the department must submit a PRF and indicate the appropriate changes.

 

Please note:

  • When a practitioner is terminated from managed care participation for 30 days or more, the application would be processed as a new applicant upon return.
  • When a practitioner's hospital privileges are resigned for 90 days or more, the application would be processed as a new applicant upon return.
