Employment or Managed Care Participation Terminations

If a practitioner voluntarily leaves UTSWMC, retires or changes to research status only, the clinical department must submit a PRF and indicate the appropriate changes.

If a practitioner terminates from participation in managed care contracts for 30 days or more, the practitioner must be processed as a new practitioner to become reinstated.
