Situations That Delay Credentialing

Practitioners must have a current Texas State license, DEA, if applicable, DPS, if applicable, and malpractice insurance in order for the credentialing packet to be considered complete.  Delay in acquiring these documents will delay the credentialing/recredentialing process.
If a practitioner's DEA is registered in another state, he/she must apply to have it transferred to Texas.

If a practitioner has malpractice history, adverse findings, and/or work history gaps, complete information must be provided during credentialing/recredentialing process, explaining each of these events.

The following information must be provided for malpractice history:
• A brief summary of practitioner's involvement in his/her own words • Date suit was filed •Current status of the suit(s) such as pending, settled, dismissed, closed • Amount of money paid on behalf of the practitioner.

• All work history gaps  must to be clarified in writing.

• If information is incomplete or insufficient for any of these situations it will delay the initial/recredentialing process.

The name on the TDI application must exactly match the name of the practitioner as it appears on the Texas State license for billing and reimbursement purposes.
