Recredentialing

1. Practitioners are recredentialed every two years.

2. The recredentialing packet, including a pre-populated application, is delivered six months before the practitioner's application is due to Credentials Committee, which is about eight months before the practitioner's current credentialing expires.

3. Due date is 45 days.
