Your Visit or Stay
Case managers are assigned to each inpatient-care area. They are trained to help patients and family members address financial, social and emotional problems that relate to illness or hospitalization. Members of the department also assist patients and families cope with long-term illnesses, rehabilitation and discharge planning.
Discharge planning begins on the day a patient is admitted to one of our hospitals. Discharge times may vary from patient to patient.

To ensure the best transition possible, we will begin preparing patients for discharge the day before they are sent home. We ask that patients and families consider and address the following prior to discharge:
- Be aware of the date and time they are to be discharged.
- Make arrangements for transportation prior to the day of discharge.
- Make sure they can pay for prescriptions.
- Be certain there is someone to assist them at home, if needed.
- Find out whether supplies and/or equipment are needed at home.
- Have all questions and concerns addressed by their physician or nurse before leaving the hospital.
