UT Southwestern Medical Center provides release of information services for patients who request medical records from UT Southwestern’s hospital or clinical practices. Patient medical records are available Monday through Friday, from 8 a.m. to 5 p.m. in adherence with all state and federal regulations for the release of information.

Patients who want to obtain a complete copy of their medical records must complete an Authorization to Disclose Protected Health Information form. Patients may download this form (pdf) or call or write UT Southwestern and one will be mailed to you. When writing, be sure to include “Request for Release of Information” on the envelope.

Complete and sign the form, then mail or bring in the original copy. Records will be delivered to the requested physician before the patient’s appointment. To obtain medical records for incapacitated or deceased patients, bring or attach a copy of the patient’s Medical Power of Attorney.
