Primary care physicians generally refer new patients to UT Southwestern Medical Center, but patients and doctors may make an appointment by using the online Request an Appointment Form or by calling 214-645-HOPE (4673) or toll-free at 866-460-HOPE (4673). They may also call UT Southwestern’s main referral line at 214-645-6455 or toll-free at 866-645-6455. In addition, doctors may also use the online Physician Referral Form or call toll-free at 866-645-5455.
Patients’ medical records are an important part of their first visits to UT Southwestern. Usually, their physicians will provide all reports, X-ray films and test results to them or UT Southwestern. Patients are asked to bring their medical records with them or verify that our staff has received the pertinent reports from their doctors.
When the patient arrives for the first appointment, our staff will ask for information such as the patient’s name, phone number, insurance provider and policy number, and the name/phone number of the referring physician. Then, during the initial examination, the physician will speak with the patient about the patient’s symptoms, condition and medical history.
After-Hours and Emergency Care
Current patients with an urgent problem or needing to see a physician immediately should call UT Southwestern at 214-645-HOPE (4673).
If a patient has a problem that is life threatening, call 911 immediately and go to the nearest emergency room; UT Southwestern’s emergency room is located at UT Southwestern University Hospital at 5909 Harry Hines Blvd. (at Inwood Road).
If a patient is taken to another hospital’s emergency room, family members should inform the emergency room staff that the individual is a patient at UT Southwestern. After the patient’s condition is stabilized, the individual can be transferred to UT Southwestern. The emergency room physician will notify UT Southwestern’s on-call doctor, who will determine the best approach for transfer.
