Study Title: Improving Outcomes in Diabetic Nephropathy

Purpose of the Study: The purpose of this research is to determine if adding on a medication to standard care prevents your kidney disease from getting worse. This study is being done because diabetes is the number one cause of kidney failure.
Participant Eligibility: 1. Adult men and women ages 20-65. 2. Type I Diabetes: Start of diabetes needing insulin before age 25 and having for at least 5 years. 3. Type II Diabetes: Start of diabetes at 20 years of age and older and being treated with oral hypoglycemic medication and/or insulin. 4. Systolic blood pressure > 130 mmHg. 5. Proteinuria in a 24 hr. urine sample while on ACE inhibitor (example: lisinopril). 6. On an ACE inhibitor or ARB medication for at least 3 months (example: lisinopril or losartan).

Other Information: This is a 12 month study for the treatment of proteinuria in Type 1 and Type 2 Diabetics. The medications being used for this study are all FDA approved. All patients will receive lisinopril and the "study" medication which will either be losartan, spironolactone or a placebo. All of the study medications, clinic visits and lab tests are at no cost to the patients.
